THIS IS ONLY FOR SAMPLE(EXAMINATION) ITEMS - IF THIS IS USED TO RETURN ORDERED ITEMS YOU WILL BE BILLED

Please have ALL cartons counted and ready for pickup in ONE easily accessible location.  

Clearly mark each box with our return address ( 6700 Sloane Dr ) if not already on box.

All returns of more than seven boxes see the instructions below:  

Click here for the Bill of Lading and fill in the date, your pickup location (shipper),  and the carton count with pounds or estimate of pounds and print this for the driver.  You do not need to save this document either when you open it or when you close it.  

Then Click here for the terminal locater and enter your pickup location zip code.  Then call the terminal and schedule a pickup.
